get values from the script

So i have a php page that i want to get the values of this script. Why does this random guy keep redirecting us to this crappy forum? jeez just tell us how to use the script it's useless if we can't get the values from form. This SHOULD be in the scope of this script because otherwise it's useless. Hell, this should be in the notes about the script.

getting value from the adding fields

I think this script is great. I am wondering if someone can tell me how you can transfer a value from the adding fields? Since you don't know how many added fields a viewer will choose, how do you transfer the data from a form to an email?

Its easy way to add/remove text box..but one suggestion how to create a label for a dynamic text box created.
